Instructions – In a Word document, answer the following questions. This assignment should be submitted to your instructor via the “Upload File” link at the bottom of this page (where your reading progress is recorded). Relapse Prevention.

Write up a relapse prevention plan for either David or a current client with early psychosis. Provide a brief paragraph providing background information if you choose to do this on a current client – but no identifying information please. Please do not submit the relapse prevention template for this exercise – instead in a Word Document, outline the following:

• the early warning signs to look out for
• the range of actions in the plan (i.e., if early warning signs are present do X, Y and Z)
• who should be involved in following through on this plan
• what are the potential barriers to following through on this plan and brainstorm on some ways to overcome these

Written work should be kept brief – 1 or 2 pages.

Once you have completed the assignment, submit it to your instructor and begin working on the next module. Please contact your instructor if you have any questions regarding the assignment.

Assignments meeting acceptable standards will be granted a “pass”. Revisions may be requested if further information or clarification is needed.
