I use this to assist me to organize what I want to accomplish for each presentation. Its also helpful to save the (evaluation) to reflect back on a similar audience, or the same one, a year later.

Another general sample. I’ve found the feedback from these to help me a great deal in tayloring my future presentations to better meet the needs of the audience. Always something to learn!
